Caring Queensland is pleased to offer Support Coordination services to our participants and their families. Caring Queensland’s Support Coordination services places the management and optimisation of your NDIS Plan in the hands of professionals who have a proven record of achieving the best outcomes for our participants or administration of an existing plan.
Participant advocacy is central to Support Coordination. Caring Queensland’s objective is to make the NDIS Plan work best for you the participant. We can do this by linking services best suited to the individuals wants and needs, connecting meaningful community lifestyle programs with the supports and education necessary to achieve your goals
Support Coordination professionals are available to present to participants and their families the complexities of NDIS planning in a user-friendly approach. Transparency at every level allows for participants and their families to have a detailed but not overwhelming understanding of exactly what is in their Plan. Finances, supports and services available and provision of technologies and assistance are administered on the participant’s behalf.
In keeping to our dedication to Person Centred Planning Caring Queensland builds respected professional relationships with participants to facilitate the individual’s best interests are carried out in their NDIS Plan.
